This is the easiest and fastest way to submit your information to become eligible to receive CEUs for your event. We must receive your completed form no later than 24 hours after the live Virtual Seminar begins. If you prefer a hard copy, a pdf is linked here. Only one submission is necessary per event.

SOW Nutritional Systems has partnered with NCCAOM, California Acupuncture Board, and Texas Chiropractic College for continuing education. It’s important and critical that you be on time and engaged throughout the virtual seminar as your attendance and participation is tracked throughout the event. Please note the time and account for your individual time zone.

• It is the responsibility of the practitioner requesting continuing education for relicensure to reference their individual state’s by-laws and to confirm seminar acceptance by their state examining board.

• Course attendance hours and participation are calculated by the webinar platform. Failure to comply with the required attendance procedures or failure to remain in the webinar may result in loss of credit hours reported.

• SOW Nutritional Systems, LLC is required by the state boards to verify attendance and participation at each seminar. Verification will be issued for the documented hours only.

• Each individual is responsible for maintaining a copy of their attendance log and certificate for their personal records, please verify with your state board in regards to how long records need to be saved, as each state may vary.
